| With a prescription from your doctor, Medicare will cover eighty (80%) percent of the
cost after the patient's annual deductible has been met. We will complete all the paper
work required by Medicare for you and collect payment directly from Medicare. We will accept private insurance assignment when the requested services are prescribed by your doctor and approved by your insurance company.
