Overview
e-homehealth offers a high-quality and competitive package of benefit programs targeted
at supporting the personal needs and financial goals of our Employees. The Company takes
pride in its commitment to provide its Employees with a quality benefits package designed
to attract, retain and reward the talented people necessary to continue as a leader in the
home medical equipment and supply products industry. This section briefly describes the
benefits the Company provides to assist its Employees in maintaining good health,
protecting their income, planning for retirement, and sharing in the success of
e-homehealth through stock ownership.
Eligibility
e-homehealth Employees who work on a full-time basis -- at least 32 hours per week --
are eligible for all benefits, after certain waiting periods. Eligibility for some
benefits, such as the e-homehealth 401(k) Savings Plan, are based upon whether you work a
certain number of hours per year, subject to certain waiting periods. If you are
interested in a specific position, you should check with the hiring manager to confirm for
which benefits you will be eligible, and when your coverage may begin.
Automatic Benefits
Once you're eligible, you receive the following benefits:
- e-homehealth recognizes the need for time away from the job for recreation, relaxation,
as well as for family needs, religious holidays, etc. Therefore, full-time hourly
Employees and management are eligible for paid vacations based on their length of service.
- Basic Life Insurance and Accidental Death and Dismemberment (AD&D) -- The Company,
at no cost, provides all full-time e-homehealth Employees with group term life and
AD&D coverage.
- Disability Income -- Some management Employees are provided long-term disability
insurance coverage at no cost. Also, depending upon employee classification, disability
benefits may include sick pay, salary continuation, and Workers' Compensation benefits.
- Family and Medical Leave -- If you need to, you can take approved Family and Medical
Leave in order to take care of yourself or a sick family member, or when a child is born
or adopted.
Elective Benefits
You can also choose the following benefits when you're eligible:
- Medical and Dental Plan -- In most areas, e-homehealth Employees can use the Preferred
Providers Organization Network of physicians and hospitals -- and save money. Some
locations have access to health maintenance organizations. Dental coverage is also
available. The Company pays the majority of the premium and the Associate pay the balance
with pre-tax contributions.
- Prescription Drug Plan -- Coverage is provided as part of the e-homehealth Medical Plan.
- 401(k) Savings Plan -- Employees can make contributions to a 401(k) savings and
retirement plan, equaling up to 15% of their pay on a pre-tax basis -- and receive a
matching contribution of 50% of the first 3 percent contributed in e-homehealth in
e-homehealth Common Stock.
- Associate Stock Purchase Plan -- All Employees with one year of service can purchase
e-homehealth Common Stock at a 15% discount through payroll deductions of 1% to 10% of
their after-tax salary.
- Short-term Disability Insurance -- All full-time Employees can purchase short-term
disability coverage.
- Flexible Spending Accounts -- Employees can set aside pre-tax dollars for certain
anticipated medical and dependent care expenses.
